Your Role:

• Provide evidence-based care for acute and chronic conditions

• Support health promotion, screening, and patient education

• Collaborate with a multidisciplinary team to improve patient outcomes

What We Offer:

• Competitive salary up to 45,000 p/annum pro rata (negotiable based on experience)

• Part-Time hours up to 30 hours p/week

Requirements:

• Registered Nurse with NMC registration

• Experience in chronic disease management (Diabetes or Asthma)

• Excellent communication and interpersonal skills

• IT proficiency and attention to detail
