Job Summary
A well-established, patient-centred medical practice in Swindon is seeking a dedicated and experienced Practice Nurse to join its dynamic healthcare team. This role is essential in providing high-quality care to a diverse patient community.

Key Responsibilities
Deliver comprehensive nursing services, including wound care, immunisations, and routine health screenings.
Conduct patient assessments, create tailored care plans, and support a holistic, patient-centred approach.
Collaborate closely with GPs, healthcare assistants, and other multidisciplinary team members to ensure continuity of care.
Provide patient education and promote health to empower individuals in managing their well-being.
Maintain accurate, up-to-date patient records, ensuring full compliance with all relevant guidelines and protocols.
Qualifications and Experience
Registered Nurse (RGN) with a valid NMC pin.
Prior experience in a primary care or community healthcare setting is highly desirable.
Skilled in administering child and adult immunizations and cervical smears.
Experience in chronic disease management is advantageous.
Strong communication skills and a collaborative approach.
Proficient in using clinical systems and electronic patient records.
