Hunter AHP have an excellent opportunity for a Complex Discharge Physiotherapist to join a fast-paced hospital discharge team in London. This role involves supporting the safe discharge of patients with complex needs, liaising with families, and coordinating care.
You ll work alongside a multi-disciplinary team, including occupational therapists, social workers, and nurses.
Key responsibilities of the physiotherapist role as follows:
- To provide a high-quality, evidence-based discharge planning service.
- To assess patients needs and coordinate appropriate support and interventions for a safe return home.
