Hunter AHP have a unique role for an Early Supported Discharge Occupational Therapist in Reading. This position involves working with patients who require additional therapy support following discharge, helping them integrate back into their home environment safely.
You ll collaborate with a multi-disciplinary team, including physiotherapists, community nurses, and rehab assistants.
Key responsibilities of the occupational therapist role as follows:
- To provide a high-quality, evidence-based early supported discharge service.
- To assess patients home environments and recommend necessary adaptations for safe discharge.
